Restitution
As a victim, you have the right to be compensated for any tangible losses that you have suffered as a result of the crime. These losses may include medical bills, funeral costs, and property loss/damage expenses. Restitution is ordered at the time of sentencing and is paid, over time, to you by the person who committed the crime. You must keep bills and receipts for items or services that were paid for because of the crime.
1. VICTIM RESTITUTION FORM (purple sheet)
Please fill out all information. This includes specific details, such as the names of hospitals and doctors from which you received care. If a question does not apply to your case, please write “not applicable” in the space. Incomplete forms cannot be processed and will be returned.
2. PHOTOCOPIES OF ANY BILLS/ESTIMATES
Please do not send the originals. If you do not have a receipt for an item, you must obtain a retail estimate and send the documentation. If your case deals with bad checks, please send along photocopies of the bad checks.
3. ALL INSURANCE INFORMATION
This includes the name of the insurance company, address, phone number, the policy number, and the name of the insured. Please send us a photocopy of any statements you receive from the insurance company. Please be aware that you are responsible for paying any balances from your statement. Balances may be claimed as restitution.
4. THE CRIME VICTIMS COMPENSATION FUND
You may be eligible to receive aid from the Crime Victims Compensation Fund if you have medical or counseling expenses, have experienced a loss of earnings or support, have suffered a cash loss of benefits, or have funeral expenses. To file a claim, contact the Crime Victims Council at (610)-437-6610 or the Victims Compensation Assistance Program at 1-800-233-2339.
